PRP or PRF: What’s the difference?

Understanding Your Platelet-Based Treatment

The main difference between PRP and PRF is that an anticoagulant is used in PRP creation to keep your blood sample from clotting while it's being treated. Fibrin (a clotting component) is permitted to develop during PRF synthesis. The finished PRF product acts as a scaffold or matrix, allowing platelets to release more slowly after your injection.

Other differences include:

  1. Amount of blood: Both PRP and PRF require blood to be taken from the patient. However, PRP needs more blood to be processed than PRF. 
  2. Types of cells: Both PRP and PRF require the whole blood sample to be placed in a centrifuge to separate the cells into distinct layers. PRF is spun at a slower speed so that some of the white blood cells and stem cells remain with the platelet layer collected for treatment. And the concentration of platelets is greater with PRF than with PRP. Thus, there are more healing factors within PRF products than with PRP
  3. Trauma to cells: In addition, the lower spin speed used for PRF causes less trauma to the individual cells, thereby making the cells more effective. 
  4. Concentration of platelets: Research shows that an ideal concentration of platelets for PRP is between 2-5 times the level of platelets typically found within the body. Newer research now points out that an even higher concentration of platelets is more effective. PRF products contain approximately 10 times the platelet concentration found within the body. Most practices are not even reaching two times native concentrations.
  5. Anticoagulant: With PRP, the blood that is collected is placed in test tubes that have an anticoagulant called acid citrate dextrose (ACD), which keeps the blood from clotting too quickly. This additive can be painful and irritating to patients. PRF does not require any anticoagulants, thereby maintaining a more natural environment for the platelets. 
  6. Fibrin: Because anticoagulants are not used with PRF, this allows the fibrinogen in the blood to convert thrombin into fibrin naturally. Fibrin, in basic terms, is what platelets bind to as they circulate. And when fibrin binds to platelets, they release growth factors that help create new collagen, skin cells, blood vessels, and nerves.

How does PRP work?

A small amount of blood is drawn from your arm. The blood is placed in a centrifuge and spun so that it separates. After 15 minutes, we take the PRP from the centrifuge and inject it into the skin. For three to ten days after your treatment, you may experience bruising, soreness, and swelling at the injection site. You should be able to resume your normal activities the next day, with a few restrictions. You are advised not to use any cosmetics, lotions, or creams on the injected area for the next six hours.

Showering should be delayed for six hours following your injection since a temperature change may hinder your body from reacting to your PRP therapy. Your provider will provide you with further information. It is crucial to follow all post-care instructions for optimal results. Your provider may recommend three sessions spaced one month apart to achieve the best effects. For best results, you'll need to return every six months for follow-up treatments.
